Prairie Ridge Hospital and Health Services offers access to our online patient portal, where you can view data from your medical records online. This information includes allergies, immunizations, active and inactive medications, current and past health concerns, and certain test results from current and past clinic visits, which will be available online after your visit.
How to Set Up Your Portal Account
If you do not currently have a login for the Secure Patient Portal, please contact us in Elbow Lake at 218-685-7300, or in Morris at 320-589-4008. You'll be asked for a current email address and then receive a link via email for further login instructions. This link will only be valid for 48 hours after it is sent.
Once you have registered, you can easily access your clinical information by visiting www.thrivepatientportal.com or by clicking on the Patient Portal link on the front page of our website.
To request access to the portal for a dependent, please fill out this Dependent Access Request Form. This form is to be filled out upon the request from a legal guardian for access to a dependent’s private health information through Prairie Ridge’s patient portal. ***Note to Legal Guardian: If the request being made is for a dependent that is a minor (ages 0-11) please be aware that access will be cut off to the guardian once the dependent turns 12 years old, per Prairie Ridge’s patient portal policy.

If you have trouble logging in or accessing the patient portal, contact the registration department Monday through Friday, 8 a.m. to 5 p.m. at 218.685.7300.
If you forget your password, you are able to reset it through the portal website. If you forget your username, you will have to call registration at 218.685.7300 to have them reset your portal. You will then have to complete the registration process again.
