Patient portal

Prairie Ridge Hospital and Health Services offers access to our online patient portal, where you can view data from your medical records online. This information includes allergies, immunizations, active and inactive medications, current and past health concerns, and certain test results from current and past clinic visits, which will be available online after your visit.

How to Set Up Your Portal Account

If you do not currently have a login for the Secure Patient Portal, please contact us in Elbow Lake at 218-685-7300, or in Morris at 320-589-4008. You'll be asked for a current email address and then receive a link via email for further login instructions. Once you have registered, you can easily access your clinical information by visiting MyCareCorner.net or by clicking on the Patient Portal link on the front page of our website.

To request access to the portal for a dependent, please fill out this Dependent Access Request Form. This form is to be filled out upon the request from a legal guardian for access to a dependent’s private health information through Prairie Ridge’s patient portal. ***Note to Legal Guardian: If the request being made is for a dependent that is a minor (ages 0-11) please be aware that access will be cut off to the guardian once the dependent turns 12 years old, per Prairie Ridge’s patient portal policy.

To request access for an Authorized Representative, please print this form and submit it to our Medical Records office.

The Prairie Ridge patient portal is related to Prairie Ridge Healthcare services only and does not include information from other medical facilities. Please call us at 218.685.7300 with questions or for assistance.
