This summary describes how medical information about you may be used and disclosed and how you can get access to this information.

Uses and disclosures of protected health information (PHI)

This document serves as notification of how Prairie Ridge Hospital and Health Services will use your PHI to support treatment, payment and healthcare operations. Your PHI may be used and disclosed by your physician, Prairie Ridge Hospital and Health Services staff, and others outside of the organization that are involved in your care and treatment for the purpose of providing healthcare services to you.

The following examples highlight the types of uses and disclosures of your PHI Prairie Ridge Hospital and Health Services is permitted to make.

  • Payment.
  • Healthcare operations.

These activities include, but are not limited to:

  • Quality assessment activities.
  • Employee review activities.
  • Training of medical students.
  • Licensing.
  • Marketing.
  • Fundraising.
  • Third-party business associates:
    • Billing services.
    • Transcription services.

Other permitted and required uses and disclosures that may be made without your consent, authorization or opportunity to object

Prairie Ridge Hospital and Health Services may use or disclose your PHI in the following situations without your consent or authorization. These situations include:

  • As required by law.
  • Public health.
  • Communicable diseases.
  • Health oversight.
  • Abuse or neglect.
  • Food and Drug Administration.
  • Legal proceedings.
  • Law enforcement.
  • Coroners, funeral directors and organ donation.
  • Research.

Uses and disclosures of protected health information based upon your written authorization

Other uses and disclosures of your PHI will be made only with your written authorization, unless otherwise permitted or required by law as described below. You may revoke this authorization, at any time, in writing, except to the extent that Prairie Ridge Hospital and Health Services has taken an action in reliance on the use or disclosure indicated in the authorization.

Other permitted and required uses and disclosures that may be made with your consent, authorization or opportunity to object

You have the opportunity to agree or object to the use or disclosure of all or part of your PHI.

If you are not present or able to agree or object to the use or disclosure of the PHI, then your physician may determine whether the disclosure is in your best interest.

Prairie Ridge Hospital and Health Services may use and disclose your PHI in the following instances:

  • Facility directories.
  • Others involved in your healthcare.
  • Fundraising activities.
  • Emergencies.
  • Communications barriers.
  • Criminal activity.
  • Workers' compensation.
  • Required uses and disclosures.

Your rights

  • You have the right to inspect and copy your PHI.
  • You have the right to request a restriction of your PHI.
  • You have the right to request to receive confidential communications from us by alternative means or at an alternative location.
  • You have the right to receive an accounting of certain disclosures Prairie Ridge Hospital and Health Services has made, if any, of your PHI.
  • You have the right to obtain a detailed copy of this notice from us.

Complaints

You may complain to Prairie Ridge Hospital and Health Services or to the Secretary of Health and Human Services if you believe your privacy rights have been violated by Prairie Ridge Hospital and Health Services. You may file a complaint with Prairie Ridge Hospital and Health Services by notifying our privacy officer either in person or in writing.
